Police are seeking urgent assistance to locate a 15-year-old boy reported missing on the Sunshine Coast on the evening of Tuesday 8 September.
The boy (pictured) was last seen leaving a medical facility and heading along Kawana Way around 6.30pm however has not been heard from since.
Police and family hold concern for his welfare as he suffers a medical condition and requires further care.
He is described as Caucasian, around 175cm tall with a slim build and short brown hair.
The boy is wearing a black jumper with “supreme” written on it in white and red, as well as dark blue tracksuit pants and grey running shoes.
Police are urging anyone who may have seen the boy in the area or know his whereabouts to come forward.
